Check off that the following have ALL been completed: *

For Retirement Only: Notify your manager three (3) months prior to your planned retirement date
Submit resignation letter to your manager/supervisor at least two weeks prior to your departure date. If you are taking another position within Partners please inform your manager
Return all keys, ID badge(s), procurement cards, parking/permit cards and/or stickers and all other company assets to your manager/supervisor
Check mail slots for any mail you may receive; mail will NOT be forwarded
Submit any outstanding business and benefit reimbursement expenses, if applicable
Cancel your T pass, if applicable
Reset phone password to the 7-digit phone number and change voicemail greeting
Check in with your manager to discuss any last day items
Update mailing/forwarding address in PeopleSoft
Set up an email "out-of-office" message as directed by manager
